Target Audience

This activity is designed specifically for primary care physicians, nurse practitioners, physician assistants, nurses, pharmacists, social workers, and other health professionals who serve patients in rural and underserved communities. 

Program Overview

Reengagement and retention in HIV care are critical components of achieving sustained viral suppression and improving overall health outcomes for people living with HIV. However, various social, economic, and systemic barriers can lead to interruptions in care. Healthcare teams must employ both clinical and community-based strategies to successfully reach individuals to continue their treatment and retain them over the long term. In this on-demand CME/CE activity, a multidisciplinary panel—including a subject matter expert, social worker, and peer educator—will lead a comprehensive discussion on the complex factors that contribute to disengagement from ongoing HIV management. The conversation will also highlight practical, evidence-based approaches to reengage and retain people living with HIV in care, as well as collaborative approaches that integrate clinical care with community resources to address the diverse needs of individuals.

Learning Objectives

Upon completion of this activity, participants should be better able to: 

  • Employ evidence-based interventions to identify, reengage, and retain people with HIV in care
  • Examine factors that impact antiretroviral therapy selection in patients with HIV such as adherence, resistance potential, and safety
  • Apply current evidence and guideline recommendations to individualize HIV treatment and meet the clinical and social needs of the patient
  • Incorporate evidence-based strategies to improve multidisciplinary management and engagement in HIV care in rural and underserved communities
