Target Audience
This activity is designed specifically for primary care clinicians, nurses, specialists in otorhinolaryngology and allergy/immunology, and other members of the healthcare team who care for patients from rural and underserved communities.
Program Overview
Team-based, coordinated care is vital for patients with chronic rhinosinusitis with nasal polyps (CRSwNP) in rural and underserved settings, where long travel distances, limited healthcare access, and complex comorbidities present challenges. Primary care clinicians, allergists/immunologists, and ENT specialists all play essential roles in treatment and ongoing monitoring, and improved synergy can help ensure optimal outcomes. This CME/CE activity, part of the four-part series entitled Clearing the Air: Collaborative Care Strategies for Chronic Rhinosinusitis With Nasal Polyps in Rural and Underserved Communities, features an interactive panel discussion amongst three interdisciplinary clinicians involved in CRSwNP management. During this conversation, they will highlight ways to work together to improve coordination and communication, optimize care transitions, and ensure adherence to guideline recommendations. They will also share insights on how to best support and engage patients throughout the care continuum, including shared decision-making and other approaches.
